# ACC Claim History Report

Follow the steps below to generate your ACC pre-employment report and **send it directly to Checkmate**.

1. Log in to your MyACC account at [my.acc.co.nz/login](https://my.acc.co.nz/login){target=`_blank`}.

2. If you don't have a MyACC account, click this [link](https://www.acc.co.nz/register-for-myacc-and-generate-a-pre-employment-report){target=`_blank`} to open the **Register for MyACC and generate a pre-employment report** page and follow the steps to register.

Do **not** use the **Register** button on the MyACC login page. You must register using the link above.

3. If you are registering for the first time, enter **Checkmate** and **checks@checkmate.tech** when you reach the following section of the registration form:

4. Once you've logged in, select the **Injuries** tab at the top of the page.

5. Scroll to the bottom of the **Injuries** page. In the **Pre-employment check** section, click **Create a pre-employment check**.

6. Enter **checks@checkmate.tech** in the email field, confirm that you've reviewed your claims list, then click **Send**. Your report will be sent directly to Checkmate.

If you didn't enter **checks@checkmate.tech** when sending your report in **Step 6**, please forward the report to **checks@checkmate.tech**.

## No ACC injuries

If you have never had an injury registered with ACC, simply follow the steps above. ACC will still generate a report confirming that no injuries are recorded.
